If you stay in Poland, you have to acquire the registration of the place of residence - so called "Meldunek" (it's necessary till 2013)
How to acquire registration of place of residence:
1) living in the dormitory (On-campus)
You have to get a form prepared by Dormitory Administration (NOTIFICATION OF TEMPORARY RESIDENCE EXCEEDING THREE MONTHS)
2) living outside compus (Off-campus)
if you rent a flat, you have to go to the Municipal Office with your house holder (don't forget to take your lease agreement).
To register at the municipal office (Urząd Dzielnicy) you have to check the competent municipal office according to the place where you live. Warsaw is divided into 17 districts and each of them has its own municipal office (Urząd Dzielnicy). The list of municipal offices in Warsaw is to download. In order to register at the municipal office, you have to go there with your Lessor /Landlord who is obliged to show the document confirming that she/he is the owner of the real estate. Moreover, she/he has to state orally that their premises is leased by you.
Finally, you will be asked to fulfill an registration motion available at the registration office.
Do not forget to take your passport or ID!
